Brief: A Community Medical Centre has an outreach staff of 5+ assorted medical professionals that often attend appointments with 
senior citizens in their homes.
1) They have decided that only one appointment per day per patient in their home is feasible and that ..
2) When patients visit the Medical Centre they should be attended by various discipline (psychology,social worker,physio',podiatrist etc)
during the one appointment wherever possible. This would cut down on time wastage and minimize the need for patients to make multiple trips. 
3) In addition they wish all patient information to be housed in one shared medical record on a central server at the medical centre.
